OUR SERVICES AIM TO CONNECT INDIVIDUALS WITH THEIR COMMUNITY AND HELP EACH PERSON REACH THEIR FULLEST POTENTIAL. DURING 2019, COMMUNITY LIVING CONNECTIONS PROVIDED SERVICES TO APPROXIMATELY 187 INDIVIDUALS WITH VARYING NEEDS AND VARYING DEGREES OF SUPPORT.1. RESIDENTIAL SERVICES: AT THE END OF2019 CLC SUPPORTED 15 INDIVIDUALS IN 4 HOMES LICENSED BY THE STATE OF WISCONSIN AS ADULT FAMILY HOMES. THESE HOMES ARE STAFFED BETWEEN 18-24 HOURS PER DAY, OFTEN WITH MULTIPLE STAFF WORKING.2. GENERAL SERVICES: AT THE END OF 2019 CLC SUPPORTED 165INDIVIDUALS; SERVICES PROVIDED RANGE FROM A FEW HOURS A WEEK TO 24 HOURS PER DAY PER INDIVIDUAL. UNDER THIS SERVICE CATEGORY, INDIVIDUALS HAVE A DAY RATE SET BY MCO'S OR FEA'S.3. CCA: AT THE END OF2019 CLC SERVED 24 INDIVIDUALS UNDER THIS DAY SERVICES CATEGORY.4.
