To support consumers with disabilities by providing information and services for independent and productive living. We provided case coordination for over 598 individual consumers this year. There were 788 measurable goals set and 352 specific measurable goals accomplished, 205 individuals received Advocacy services, 242 individuals received Assistive Technology service, 91 individuals received Housing services, 73 individuals received Independent Living and Life Skills, 238 individuals received Peer Consoling services, and 6 individuals received Person Care services. A total of 5 individuals transitioned from nursing homes to personal residences. Diversion services prevented 23 individuals from being placed in an institution. Number of individuals received vocational services 48 and transition age youth, ages 18 to 26, 41.
