It achieves the triple aim with 90% customer satisfaction, 67% decrease in hospital admissions, an estimated $16 million saved with a 5:1 return on investment. At home, the clinical, social and behavioral care team works closely with patients for six months to provide access to quality care and social determinates of health solutions like nutritional food and housing. PHII builds upon Dr. Jeffrey Brenner's theory of hot spotting for integrating comprehensive medical, social and mental health services into the community health needs of ethnically diverse patients. The center for Medicaid and Medicare acknowledged PHIII sustainable business model using fee-for-service contracts with health providers.
