In 2018, hospice services of Lake County provided end of life care to 272 patients and their families. This care was provided by a team of registered nurses, social workers, hospice aides, spiritual caregivers, volunteers, and bereavement counselors. RN caregivers planned, organized and directed patient care under the physician's orders. The RN case manager facilitated the patients' efforts toward optimal comfort care, acquisition of appropriate medical equipment and supplies, medication management, dietary counseling, home safety, and long term planning. Hospice aides provides personal care and assistance with the activities of daily living such as feeding and bathing. They also provided limited household services that maintain a safe and sanitary environment in the areas of the home used by the patient.
