Medical: SNAHC provides services to individuals and families across all lifecycles including primary, pediatric, and preventive health care, immunizations, management of acute and chronic medical concerns and problems (e.g.: diabetes, hypertension, hepatitis C treatment, transgender hormone therapy), medication monitoring, health assessments, health education, advocacy, and specialty services including optometry, podiatry and chiropractic care. SNAHC is a Title X grantee through the Office of Population Affairs and a certified family planning, access, care, and treatment (Family PACT) provider.
