HOUSING 2006 OWNS TWO, 6-BED GROUP HOMES IN ADAMS COUNTY, ILLINOIS. THESE GROUP HOMES ARE HOUSING AND URBAN DEVELOPMENT (HUD) SPONSORED, MAKING IT POSSIBLE FOR RESIDENTS TO RESIDE IN THE HOMES ACCORDING TO THEIR INCOMES. TRANSITIONS OF WESTERN IL, A NOT-FOR-PROFIT PROVIDER OF MENTAL HEALTH, REHABILITATION, AND EDUCATIONAL SERVICES, PROVIDES STAFFING AND SERVICES IN THE HOMES OWNED BY HOUSING 2006. DURING FY17, 12 ADULTS WITH A DEVELOPMENTAL DISABILITY RESIDED IN THE TWO HOMES OWNED BY HOUSING 2006. THESE 12 RESIDENTS RECEIVED 24-HOUR CARE AND SUPERVISION FROM TRANSITIONS' STAFF. RESIDENTS LEARNED SKILLS OF INDEPEPNDENT LIVING - SKILLS, SUCH AS GROOMING AND HYGIENE SKILLS, SOCIAL SKILLS, LAUNDRY SKILLS, AND RECREATIONAL/LEISURE SKILLS - THROUGH THE MILIEU IN THE HOMES. STAFF OF THE HOMES PROVIDED MEALS AND ASSISTED RESIDENTS WITH PERSONAL NEEDS, SUCH AS MEDICATION MANAGEMENT, TRANSPORTATION IN THE COMMUNITY, ATTENDING APPOINTMENTS WITH OTHER PROVIDERS (MEDICAL AND SOCIAL SERVICE), AND SELF-CARE. WITH AFFORDABLE HOUSING PROVIDED BY HOUSING 2006 AND THE SUPERVISION AND SUPPORTS PROVIDED BY TRANSITIONS, ADULTS WITH A DEVELOPMENTAL DISABILITY CAN LIVE IN THE COMMUNITY, CLOSE TO FAMILY AND FRIENDS, RATHER THAN LIVE IN AN INSTITUTIONAL SETTING.
