His Branches Health - His Branches is uniquely neighborhood-centric, with two practice locations in Rochester New York: Grace Family Medicine in the 19th Ward, and Joy Family Medicine in the Beechwood neighborhood. In our 2018/2019 fiscal year we served 1,991 patients, of all ages, through 6,804 patient encounters. In caring for the whole person, we address all aspects of well-being: body, mind, and spirit. We specialize in meeting complex needs and removing barriers to good health. We offer group learning in chronic conditions, on-site lab services, care management, and case management to facilitate the connection to much needed social support services.This program is supported by contributions.
