QUALITY IMPROVEMENT AND POPULATION HEALTH: CHF SUPPORTS PEDIATRICIANS IN ENHANCING OR FORMALIZING THEIR CARE DELIVERY MODELS TO SUPPORT EFFECTIVE OPERATION AS A PEDIATRIC PRIMARY CARE MEDICAL HOME. THE ABILITY TO TARGET SPECIFIC POPULATIONS OF CHILDREN AND YOUTH WITH SPECIAL HEALTH CARE NEEDS (CYSHCN) IS KEY IN FOCUSING PRACTICE RESOURCES ON CHILDREN AND FAMILIES WITH THE MOST NEED AND ACHIEVING GREATEST IMPACT. CHF HAS DEVELOPED A PEDIATRIC POPULATION MODEL TO IDENTIFY POPULATIONS BASED ON MEDICAL AND PSYCHO-SOCIAL CONDITIONS WHICH CAN CORRELATE WITH THE LEVEL OF NEED FOR CARE COORDINATION/MANAGEMENT. CHF HAS ALSO ENHANCED THE FUNCTIONS OF A POPULATION HEALTH MANAGEMENT TOOL TO FOCUS ON THE NEEDS OF THE PEDIATRIC POPULATION. CHF CONTINUES TO DEVELOP SPECIFICS FOR THE PEDIATRIC POPULATION AND HAS TRAINED PRACTICES ON METHODS TO UTILIZE THE INFORMATION TO BETTER UNDERSTAND THE HEALTH OF THEIR POPULATION. THE SCOPE OF OPPORTUNITY FOR PEDIATRIC CARE COORDINATION SPANS ACROSS MANY SETTINGS INCLUDING MEDICAL CARE, MENTAL HEALTH SERVICES, COMMUNITY PROGRAMS, SCHOOL SYSTEMS AND FAMILY/CARE TAKER SETTINGS. CHF ALSO SUPPORTS PRACTICES IN QUALITY IMPROVEMENT METHODS, SPECIFICALLY FOCUSING ON TWO MAIN AREAS IMPACTING PEDIATRIC HEALTH: ASTHMA AND IMMUNIZATIONS. TO SUPPORT IMPROVEMENT EFFORTS IN ASTHMA, CLINICAL SUPPORT TOOLS AND TRAINING TO SUPPORT THE DELIVERY OF "PERFECT ASTHMA CARE" ARE BEING DELIVERED AND ADOPTED BY PARTICIPATING PEDIATRIC PRACTICES. IN ADDITION, CHF CONTINUES TO PROVIDE TRAINING AND EDUCATION ON METHODS TO IMPROVE IMMUNIZATION RATES FOR TWO-YEAR-OLDS AND ADOLESCENTS. THESE EFFORTS INCLUDE COMMUNITY EDUCATION PROGRAMS, PEDIATRICIAN TRAINING, AND CLINICAL SUPPORT TOOLS.
