Practice transformation - with support from the Centers for Medicare and Medicaid Services (CMS), the Transforming Clinical Practice Initiative (TCPI) is designed to assist more than 140,000 clinician practices from 2015-2019 in sharing, adapting, and further developing of comprehensive quality improvement strategies. As a TCPI awardee, PCPCC works to support practice improvement teams though its diverse network of stakeholders to foster partnerships with patients, family caregivers, and community-based organizations to achieve common goals of improved care, better health and reduced costs.
