Jewish Home's certified home health agency ('CHHA') opened in 2013. The CHHA provides home-based care and rehabilitation to those recovering from an illness, injury or hospitalization throughout the five boroughs of New York City and Westchester, Rockland Nassau and Suffolk counties. CHHA care teams help clients get well and stay well at home with the goal of remaining out of the hospital if at all possible. Licensed by the New York State Department of Health, the program serves patients with Medicare or patients who are dual eligible with Medicare and Medicaid. The program also serves patients in managed care plans services are available based on assessed needs and include: - nursing physical, occupational & speech therapy - home health aides - social services nutrition - psychiatric nursing - wound care - diabetes education.
