SMH Case Management provides case management services for medically fragile children through the Care at Home Program. In addition, Case management services are provided to health home eligible children. Health home case managers coordinate patient services and promote access to resources that help to improve health outcomes, reduce preventable hospitalization and ER visits and avoid unnecessary care for Medicaid members. In 2018, New York State Office for People with Developmental Disabilities ("OPWDD")changed the Medicaid Service Coordination ("MSC")model. MSC services operating under CMC were transitioned to a designated Care Coordination Organization/Health Home. As a result, CMC is no longer providing MSC services.
