Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a member of Empath Health, a nonprofit integrated network of care supporting those challenged by chronic and advanced illness. We aim to fully support and care for seniors facing chronic illnesses and aging. Suncoast PACE (program of the all-inclusive care for the elderly - a nursing home diversion program) provides a full range of home and health care services to seniors 55 years of age or older. PACE allows its participants to continue to live in their homes and maintain their independence.
