Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a member of Empath Health, a nonprofit integrated network of care supporting those challenged by chronic and advanced illness. We aim to fully support and care for seniors facing chronic illnesses and aging. Our team helps our participants achieve a healthy quality of life, stay active and socially connected and live safely and independently at home and in the community.

Suncoast PACE (program of the all-inclusive care for the elderly - a nursing home diversion program) provides a full range of home and health care services to seniors 55 years of age or older. PACE allows its participants to continue to live in their homes and maintain their independence. The program offers primary & specialty medical care, adult day center services, hospitalization & emergency care, nutritious meals, in-home support and care, physical occupational & speech therapies, transportation and pharmacy services.
