Accomplishments during FY2019 include: CTC has successfully executed year 2 of its 5 year strategic plan. CTC has actively engaged a new cohort of 17 new pediatric practices to our multi-payer, patient centered medical home transformation program called PCMH-Kids. In addition, we have engaged 30 adult and pediatric primary care practices in implementing integrated behavioral health. Approximately 650,000 Rhode Islanders (2/3 of the state population) receive their care from practices that participated in CTC-RI and PCMH-Kids transformation program. PCMH-Kids now represents more than 50% of Rhode Island children (110,000 patients), and more than 80% of the state's total pediatric Medicaid population. To date, CTC has worked with 128 adult and pediatric primary care practices, representing over 800 primary care providers.
