About
Funded operational subsidies for five primary care centers providing medical and dental services to underserved population in West Virginia under a community health centers grant from the U.S. Department of Health and Human Services. Additionally, funded operational subsidies for five primary care centers providing medical, dental, optometry and behavioral services to underserved population in West Virginia and bordering Kentucky and Ohio counties under a community health centers grant from the U.S. Dept. of Health and Human Services. Other services provided to patients include: nutritional, diabetes education, social determinant services, patient advocacy services, black lung program, adolescent health care program, school based health care services, substance use disorder services, transitional care management, chronic care management regardless of a patient's ability to pay. Additionally, financial assistance to eligible patients for pharmaceuticals are provided, as well as health care for the homeless. Occasional screenings for vision and dental are conducted, along with free blood pressure checks, and significant educational training is provided to physician, pharmacy, and psychology residents and nurse practitioners. Also provided $187,216 in charity care to indigent patients.
