IS A PRIVATE NON-PROFIT AGENCY FOR AT-RISK ADOLESCENTS AND THEIR FAMILIES, OFFERING THE FOLLOWING PROGRAMS: RESIDENTIAL: 141 ACRE CAMPUS-BASED PROGRAM IN CONYERS GEORGIA OFFERING SAFE HOUSING; EDUCATION; GED AND VOCATIONAL TRAINING; INDIVIDUAL, GROUP AND FAMILY THERAPY; HEALTH AND WELLNESS CLASSES; LIFE SKILLS DEVELOPMENT; JOB SKILLS DEVELOPMENT AND A PREPARATION FOR INDEPENDENT LIVING CURRICULUM. THE CENTER OPERATES 24 HOURS PER DAY, 7 DAYS A WEEK, 365 DAYS EACH YEAR. TRANSITIONAL LIVING: OFFERS CAMPUS-BASED APARTMENT LIVING FOR YOUTH TRANSITIONING FROM FOSTER CARE TO INDEPENDENT LIVING; SUPPORT SERVICES ARE PROVIDED DURING THE TRANSITION PERIOD. THERAPEUTIC FOSTER CARE: THOROUGHLY SCREENED AND HIGHLY TRAINED FOSTER FAMILIES ARE USED TO MEET THE SPECIALIZED NEEDS OF FOSTER YOUTH, FROM AGES 4-18, IN A FAMILY SETTING. THESE SERVICES ARE PROVIDED AT THE CONYERS CAMPUS AND OFFICES LOCATED IN DALTON, SAVANNAH, AND VALDOSTA, GEORGIA. INTAKE AND ASSESSMENT: THE ORGANIZATION HAS CENTRALIZED INTAKE FOR ALL OFFICES. REFERRALS ARE RECEIVED FROM THE STATE OF GEORGIA DIVISION OF FAMILY AND CHILDREN SERVICES ("DFCS") OFFICES ALL OVER THE STATE OF GEORGIA. AS A TEAM, IT IS DETERMINED IF THE NEEDS OF THE YOUTH CAN BE MET BY THE ORGANIZATION'S PROGRAMS AND A PLACEMENT CLOSEST TO THEIR HOME COUNTY IS IDENTIFIED. THE YOUTH'S INFORMATION IS THEN STAFFED WITH THE DIRECTOR AND DFCS CASEWORKER IS INFORMED THAT A PLACEMENT HAS BEEN FOUND.
