In 2017 hospice care team provided 382 nursing visits and additional continuous care nursing visits. Hospice offers care that provides for the patient's emotional, spiritual and physical needs as well as for the patient's family. Care is generally provided in the patient's home while putting the patient and their family at the center of the care plan process. An individualized plan of care is created utilizing the hospice care team medical directors, nurses, aides, chaplains, social workers, bereavement coordinator and trained volunteers to provide comprehensive, compassionate care for patients nearing the end of life. Our services include pain and symptom management, emotional and spiritual support, respite for family caregivers, medications, equipment, supplies and grief support for loved ones. Services are provided whether the patient is living or receiving care. It may be in a nursing home, hospital, assisted living facility, or in their patient's home. Hospice care team works with patients, families and their care providers to insure a seamless continuum of quality end of life care.
