DURING THE YEAR ENDED JUNE 30, 2018, THE ORGANIZATION CONTINUED TO STRIVE TO PROVIDE HIGH-QUALITY, AFFORDABLE HEALTH CARE TO LOW-INCOME FAMILIES IN SANTA CLARA COUNTY. SCHOOL HEALTH CLINICS PROVIDED 16,245 HEALTH CARE VISITS TO 6,322 PATIENTS ACROSS ALL FIVE SCHOOL HEALTH CLINIC SITES. DURING THE YEAR ENDED JUNE 30, 2018, SCHOOL HEALTH CLINICS OF SANTA CLARA COUNTY UTILIZED ITS RESOURCES WITHIN QUALITY IMPROVEMENT TO UNDERTAKE IMPROVEMENTS IN OUR CURRENT PRACTICE TRANSFORMATION MODELS TO ACHIEVE PATIENT-CENTERED MEDICAL HOME LEVEL III RECOGNITION AT ALL FIVE CLINIC SITES. THIS DESIGNATION NOT ONLY RECOGNIZES THE MODELS OF PATIENT-CENTERED CARE THAT HAVE BEEN PUT IN PLACE WITHIN THE ORGANIZATION BUT IS ALSO RECOGNIZED BY FUNDERS AND ALLOWS FOR INCREASED GRANT OPPORTUNITIES. THE ORGANIZATION WAS THE RECIPIENT OF ADDITIONAL GRANTS IN FISCAL 2018 FROM THAT WERE NOT AWARDED IN THE PAST, INCLUDING ONE FROM CISCO AND NEW HRSA FUNDING. CISCO SUPPORTED SCHOOL HEALTH CLINICS OF SANTA CLARA COUNTY TO PROVIDE HEALTH EDUCATION IN SEVERAL SANTA CLARA COUNTY SCHOOLS. THE HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) GRANTED FUNDING TO EXPAND BEHAVIORAL HEALTH SERVICES, INCREASE SUBSTANCE ABUSE SCREENING AND TREATMENT, AND TO IMPROVE TECHNOLOGY THROUGH THE ACCESS INCREASES IN MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES (AIMS) GRANT AWARDED TO SHC IN SEPTEMBER 2017.SCHOOL HEALTH CLINICS ALSO IMPROVED ACCESS FOR HUNDREDS OF FAMILIES IN SANTA CLARA COUNTY BY PARTNERING WITH KAISER PERMANENTE SAN JOSE AND THEIR FAMILY RESIDENCY PROGRAM. IN EARLY 2018, THE FAMILY RESIDENCY PROGRAM BEGAN TO ROTATE THEIR RESIDENTS INTO OUR WASHINGTON NEIGHBORHOOD HEALTH CLINIC SITE PROVIDING MORE HEALTH CARE VISITS TO THOSE IN OUR CLINICS. THE EXPERIENCE IN-TURN ALLOWS FUTURE FAMILY PRACTICE PHYSICIANS TO LEARN WHAT IT IS LIKE TO PROVIDE COMMUNITY-BASED HEALTH CARE SERVICES FOR A POPULATION WITH SIGNIFICANT HEALTH AND SOCIOECONOMIC NEEDS.
