Certified home health agency. In 2018, the UHHS/CHHA served 1,129 patients with 24,834 visits. The certified home health agency (CHHA) offered skilled, part time services to patients in need of rehabilitation, skilled nursing and observation and teaching to restore them to their previous level of functioning, if possible. Many patients come directly from a hospital setting, or they may be discharged to the CHHA from a skilled nursing facility, where they have already gone through some rehabilitation and skilled care. Patients also may be referred directly from their provider, who notes they need something additional in the home setting to help them with strength, some sort of healing or education, medication management, or some other skilled services on a temporary basis. The CHHA offers skilled nursing services from RNs and LPNs, physical, occupational and speech therapy, nutritionist services, social worker services, and home health aide help in the home. Most CHHA cases are short term, with the goal of getting the patient back to the most independent level possible. The CHHA contracts with Medicare, Medicaid, all managed plans for both, work comp cases, no fault, or commercial insurance for skilled patient care. Most of the work done by clinicians involve close monitoring of patient status through frequent visits, other clinician and aide visits, and regular discussions with the provider in the community, who orders the care. The objective of the program in most cases is to keep the patient safe at home, and restore them to as close to the previous level of functioning as possible. A few patients in the program stay on for years, as to discharge them would lead to a decrease in their level of functioning.
