PROJECT HOME PROVIDES HOUSING, EDUCATION, EMPLOYMENT, MEDICAL AND SUPPORT SERVICES FOR FORMERLY HOMELESS INDIVIDUALS AND FAMILIES AS WELL AS OUTREACH EFFORTS FOR INDIVIDUALS LIVING ON THE STREETS. THE ORGANIZATION HAS DEVELOPED A PROVEN AND EFFECTIVE PROGRAM TO ASSIST PERSONS IN OVERCOMING CHRONIC HOMELESSNESS. THIS "CONTINUUM OF SERVICES" IS CAREFULLY DESIGNED TO DEAL WITH THE COMPLEX ISSUES OF PERSONS WITH SPECIAL NEEDS SUCH AS MENTAL ILLNESS AND ADDICTION. THE SERVICES CONSIST OF STREET OUTREACH; A RANGE OF SUPPORTIVE HOUSING FROM ENTRY-LEVEL TO PERMANENT, LEASE BASED HOUSING, COMPREHENSIVE SERVICES INCLUDING HEALTH, BEHAVIORAL HEALTH AND DENTAL CARE SERVICES AND EMPLOYMENT AND EDUCATION SERVICES; AND ADVOCACY AND STRATEGIC INITIATIVES.PROJECT HOME'S HOUSING AND SUPPORTIVE SERVICES OFFER PERMANENT, SUBSIDIZED, SUPPORTIVE HOUSING FOR INDIVIDUALS AND FAMILIES WHO HAD BEEN HOMELESS. CURRENTLY, WE HAVE DEVELOPED 780 UNITS OF SUPPORTIVE AND AFFORDABLE HOUSING FOR PERSONS WHO HAVE EXPERIENCED HOMELESSNESS AND LOW-INCOME PERSONS AT RISK OF HOMELESSNESS. OUR GOAL IS TO COMPLETE 1,000 TOTAL UNITS OF AFFORDABLE HOUSING IN THE NEAR FUTURE. PROJECT HOME'S OUTREACH COORDINATION CENTER (OCC), IN PARTNERSHIP WITH THE CITY AND OTHER SERVICE PROVIDERS, COORDINATES OUTREACH TO PEOPLE LIVING ON THE STREETS OF PHILADELPHIA. THE OCC REACHES OUT TO OVER 2,200 UNDUPLICATED INDIVIDUALS ANNUALLY.
