THE COMMUNITY HEALTH CENTERS OF BURLINGTON (CHCB) IS A PRIMARY CARE FAMILY PRACTICE AND A FEDERALLY QUALIFIED HEALTH CENTER OFFERING MEDICAL, DENTAL, LABORATORY, PSYCHIATRY, COUNSELING, SUBSTANCE ABUSE AND CASE MANAGEMENT SERVICES AS PART OF AN INTEGRATED HEALTH CARE HOME. CHCB SERVES ALL MEMBERS OF THE COMMUNITY REGARDLESS OF LIFE CIRCUMSTANCE, INCOME OR INSURANCE STATUS. CHCB OFFERS A SLIDING-FEE SCALE FINANCIAL ASSISTANCE PROGRAM FOR ALL SERVICES BASED ON INCOME, AND AT NO COST TO THOSE QUALIFIED AS HOMELESS. CHCB'S EIGHT HEALTH CENTERS ARE LOCATED IN CHITTENDEN AND SOUTHERN GRAND ISLE COUNTIES. THE FOLLOWING ARE A FEW OF THE FISCAL YEAR 2019 HIGHLIGHTS THAT CHCB HAS ACHIEVED: -CHCB ENDED THE FISCAL YEAR PROVIDING A RECORD-SETTING 129,101 PATIENT VISITS. -A TWO-MONTH EXTENSION WAS PILOTED FOR THE LOW-BARRIER SHELTER THIS YEAR, FUNDED BY THE CITY OF BURLINGTON. UP TO 37 INDIVIDUALS EXPERIENCING HOMELESSNESS WERE ABLE TO RECEIVE SHELTER, A WARM MEAL AND CONNECTION TO WRAP-AROUND SUPPORT SERVICES EVERY NIGHT FROM NOVEMBER 1, 2018 THROUGH JUNE 15, 2019 - REGARDLESS OF SOBRIETY OR MENTAL HEALTH CHALLENGES. -KNOWING THAT NEW AMERICANS AND LIMITED ENGLISH COMMUNITY RESIDENTS OFTEN STRUGGLE WITH HISTORIES OF TRAUMA AND COMPLEX MEDICAL CONDITIONS, CHCB PROVIDED 12,707 INTERPRETER-ASSISTED VISITS TO ENSURE CULTURALLY COMPETENT CARE. -AS ONE OF THE LARGEST FAMILY PRACTICES OFFERING MEDICATION ASSISTED TREATMENT, CHCB NOW CARES FOR OVER 475 PATIENTS RECEIVING SUBOXONE TREATMENT AS PART OF VERMONT'S ONGOING FIGHT AGAINST OPIOID ADDICTION. -CHCB PILOTED A NEW CHILD PSYCHIATRY CONSULTATION CLINIC, WORKING WITH AREA PEDIATRICIANS AND OTHER PRIMARY CARE PROVIDERS TO HELP ALLEVIATE THE SEVERAL MONTHS-LONG WAITLIST OF CHILDREN NEEDING TO BE SEEN BY PSYCHIATRISTS IN VERMONT. -WITH PHILANTHROPIC SUPPORT FROM THE UNITED WAY OF NORTHWEST VERMONT AND THE HEALTHY SMILES, HEALTHY CHILDREN FOUNDATION, CHCB HIRED THEIR FIRST DENTAL CASE MANAGER POSITION. THIS POSITION WORKS WITH FAMILIES TO REMOVE BARRIERS TO BRINGING IN THEIR CHILDREN FOR CRITICALLY-NEEDED DENTAL CARE. -CHCB ADDED A FULL-TIME LICENSED CLINICAL SOCIAL WORKER TO THEIR MATERNAL/CHILD HEALTH TEAM. EACH YEAR, THIS PROGRAM CARES FOR OVER 170 PREGNANT WOMEN, PRIMARILY THOSE WHO ARE LOW INCOME, WITH LOW LITERACY, AND A HISTORY OF SUBSTANCE ABUSE, MENTAL ILLNESS OR TRAUMA. -RECOGNIZING THE NEED FOR INCREASED ACCESS TO CARE FOR HOMEBOUND INDIVIDUALS, CHCB BEGAN OFFERING TELE-PSYCHIATRY AND TELE-THERAPY APPOINTMENTS. JUST IN ITS PILOT STAGE, CHCB IS EXPLORING HOW TO EXPAND THIS INNOVATIVE CARE.
