The mission of Friend Family Health Center (FFHC) is to provide access to quality, patient-centered primary and preventive health care services regardless of an individual's socioeconomic status. FFHC utilizes a 'medical home' model of care that facilitates a continuous relationship between the patient, the patient's family, and the patient's personal primary care provider. The medical home model consists of a care team working collaboratively to help improve the quality, effectiveness, and efficiency of care delivery while responding to each patient's unique needs and preferences. To ensure that FFHC is responding directly to the needs of those served, more than half of the health center's board of directors are active patients and live or work in its approved service areas.
