As a participant of Medicaid HCBS programs, the HPC is now eligible to work with the comprehensive personal care services for the Frail Elderly (HCBS/FE), the Physically Disabled (HCBS-PD), Developmentally Disabled, and the Traumatic Brain Injury Waiver (HCBS-TBI). Our consumers are primarily persons who are at risk of hospitalization or institutionalization, such as the elderly, chronically ill and disabled.
