SUBSTANCE ABUSE SYSTEMS:REACH Health Services is a community based comprehensive outpatient substance use disorders treatment program designed to deliver outpatient, intensive outpatient, health home case management and population based health services and pharmacotherapy services to those with substance use disorders in Baltimore City and the surrounding counties. The program offers group and individual counseling services six days per week. The program expands access to care through the development of partnerships with existing psychosocial programs in their communities and coordinates the care with these other agencies. Current services include assessment and treatment planning, pharmacotherapy, limited somatic care, case management, referral, and group and individual counseling services. Additional service provision is enhanced through a close collaboration with local health care providers. No persons meeting Federal admission criteria will be denied admission based on race, religion, marital status, sex, age, ethnic origin, disabilities or sexual preference.REACH continued the Hub and Spoke model of care (grant funded) to the continuum of care services we already provide. The Hub and Spoke model allows us to take patients from hospitals, other health care facilities and off the street, stabilize them on medication assisted treatment (buprenorphine) and connect them with primary care facilities in their communities for ongoing medication assisted treatment. REACH may continue to provide counseling services for this population or they may come back to us for more intensive treatment and observed medication for a time period if they de-stabilize in the community. Reach Health Services is currently the recipient of a Federal SAMHSA grant which focuses on the recruitment of African American women and LGBTQ patients with substance use disorders at risk for Hepatitis C and/or HIV. The grant allows us to provide testing and treatment for Hepatitis C and testing and referral for care coordination and case management for HIV care. REACH partners with community agencies for the care coordination and referral. REACH has continued to provide updated and new LGBTQ cultural competency training for staff. REACH has hosted the Federal Bureau of Prisons psychologists, pharmacists and physicians to discuss the best practice interventions for the implementation of medications used to treat opioid use disorder in the Federal Prison System.REACH Health Services completed an in-depth HIPAA vulnerability analysis with our IT company and developed new policies and procedures to address passwords, access to certain systems, etc. REACH Health Services also completed a 3 day accreditation process and successfully received national accreditation from the Council on Accreditation of Rehabilitation Facilities (CARF).
