Mission
NBHP provides a forum for the leadership of Houston's mental health and substance use providers, both public and private, to communicate, coordinate, and collaborate to improve the community's behavioral health system.
About
NBHP initiated a major collaborative, the "Community Coordination of Care (C3) Initiative" that has over 100 participants representing over 60 greater Houston agencies and organizations. The purpose of the effort is to develop and implement a model of care coordination that includes primary care, behavioral health care, and social service providers that uses social determinants of health model as its conceptual approach.
