Key programs include Highline Health Connections: Care navigation for vulnerable populations a program that bridges gaps in the healthcare system to improve the health of the most vulnerable in our community. This program serves over utilizers of our Emergency Department who have chronic medical conditions, who are low income and who many times, also have mental and behavioral health issues. A care management team address the needs of each Health Connections participant, supporting the participant to eliminate barriers, create a road map to success and achieve the ultimate goal of staying out of the hospital and taking initiative in their own health and well-being. In its third year, the program has served over 800 people and continues to be successful in exceeding expectations, cutting hospital costs and mostly importantly, it continues to produce positive patient outcomes. Other focus areas of Foundation support for Highline Medical Center include nursing advancement and engagement, medical innovation and emerging and critical need. The Foundation offers scholarships and support for nurses and doctors who are continuing their education or who are looking for professional development. In an effort to hire and retain the best and brightest talent, the Foundation encourages Highline staff to apply for annual scholarships and financial aid for certificate programs and continued education. The Foundation supports medical innovation through funding technologically advanced equipment such as a UV cleaning robot, which helps drastically reduce hospital acquired infection rates. And last, the Highline Medical Center Foundation supports emerging and critical need, such as ongoing support of our award-winning cancer center through the provision of direct patient support such as prescriptions, emergency housing, or gas and food cards in financial emergency situations.
