The Center for Well-Being’s (CWB) mission is to promote the well-being of the whole person by empowering people with the knowledge, skills, and spirit to take responsibility for personal, family, and community health. CWB serves residents through community partnerships that focus on preventing chronic disease through education, mobilization and advocacy. Our programming empowers individuals through behavioral change and self-efficacy related to diabetes, heart disease, obesity, smoking, chronic pain, and substance abuse. As the primary free nonprofit health education resource in Sonoma County, we reach over 20,000 residents annually through workshops, exercise classes, workplace wellness, school programs and community outreach.

Program Services: The Center for Well-Being is transforming community health through innovative partnerships and high-quality chronic disease prevention services that are integrated with our healthcare partners. As a respected community-based health education resource, we are also focusing upstream to make changes at the local level, supporting policies that reduce tobacco exposure and infrastructure changes that promote access to physical activity for our children. We are committed to improving community health through high-quality, cost-efficient services. Center HAS THE ONLY DIABETES PROGRAM IN SONOMA COUNTY THAT IS ACCREDITED BY THE AMERICAN DIABETES ASSOCIATION AND OPERATES THE ONLY CARDIAC REHABILITATION FACILITY IN SANTA ROSA- HEARTWORKS. OUR PROGRAMS SERVE OVER 14,000 INDIVIDUALS WITH CHRONIC HEALTH CONDITIONS EACH YEAR, MANY OF WHOM HAVE A LOW TO MODERATE INCOME AND ARE IN GREAT NEED OF HEALTH EDUCATION. OUR NON-PROFIT IS PRIMARILY FEE-FOR-SERVICE,WITH APPROXIMATELY 87% OF OUR CLIENT BASE ACCESSING SLIDING SCALE OR FREE SERVICES.WHILE NCCWB HAS BEEN DEEPLY AFFECTED BY BUDGET CUTS FROM THE STATE OF CALIFORNIA, OUR BOARD OF DIRECTORS IS DEDICATED TO PROTECTING OUR CORE SERVICES THAT LEAD TO REDUCED HOSPITALIZATIONS AND IMPROVED QUALITY OF LIFE. COMMUNITY CONTRIBUTIONS SUPPORT OUR ABILITY TO OFFER FREE OR LOW-COST SERVICES TO THOSE WHO HAVE DIFFICULTY AFFORDING HEALTHCARE.
