ACTIVITIES INCLUDE HELPING INDIVIDUALS TO OBTAIN HEALTH INSURANCE THROUGH THE NYS MARKETPLACE; ASSISTING INDIVIDUALS TO OBTAIN HEALTH CARE SERVICES AND OTHER RELATED SERVICES; SPONSORING EDUCATIONAL EVENTS AND ACTIVITES FOR COMMUNITY MEMBERS AND PROFESSIONALS.PROGRAMS INCLUDE:WHOLE HEALTH ACTION MANAGEMENT PROGRAM - 8 SESSIONS HELDCHRONIC DISEASE SELF MANAGEMENT, CHRONIC PAIN SELF MANAGEMENT & DIABETES SELF MANAGEMENT PROGRAMS - 29 SESSIONS HELDDELIVERY SYSTEM REFORM INCENTIVES PAYMENT (DSRIP) PROJECTS - 470 PATIENT ACTIVATION MEASUREMENT (PAM) SURVEYS, 272 NAVIGATION SERVICES, 634 CARE TRANSITIONS SERVICES, 40 NEED ASSESSMENTS, 246 BEHAVIORAL HEALTH SCREENINGS, AND 195 PARTICIPANTS TRAINED IN MENTAL HEALTH FIRST AID.HEALTH INSURANCE ASSISTANCE PROGRAM - 2,422 COMPLETED/PARTIALLY COMPLETED ENROLLMENTSCOMMUNITY HEALTH ADVOCATE PROGRAM - 654 CASESFINANCIAL ASSISTANCE PROGRAM - 16 INDIVIDUALS SERVED; 123 UNITS OF SERVICE PROVIDEDPRESCRIPTION ASSISTANCE PROGRAM - 50 INDIVIDUALS SERVED; 318 UNITS OF SERVICE PROVIDEDPOPULATION HEALTH IMPROVEMENT PROGRAM - ACTIVITIES TO ADDRESS THE NEEDS OF CHENANGO COUNTY RESIDENTS INCLUDING WORKPLACE WELLNESS, TRANSPORTATION AND THE DENTAL INITIATIVE WHICH IMPACTED ROUGHLY 4,000 CHENANGO COUNTY STUDENTS FROM PRE-K THROUGH 5TH GRADETOBACCO FREE COMMUNITIES AND DRUG FREE COMMUNITIES PROGRAMS ALONG WITH THE CHENANGO SUBSTANCE ABUSE PREVENTION COALITION - SPONSORED EDUCATIONAL EVENTS, AND ACTIVITIES TO EDUCATE CHENANGO COUNTY RESIDENTS AND WORKED TO PROMOTE A CLEAN, SAFE AND ADDICTION-FREE COMMUNITY.
