Routine Home Care: When the patient who has elected to receive hospice care is in a home setting such as a private residence, long term care facility, adult group home, or assisted living and is not receiving continuous home care. Continuous Home Care: when an individual who has elected to receive hospice care is not in an inpatient facility and receives hospice care consisting predominantly of nursing care on a continuous basis in a home setting. Continuous home care is only provided during brief periods of crisis and only as necessary to maintain the terminally ill patient in the home setting and avoid inpatient placement. 42 C.F.R. 418.2049A General Inpatient Care: When an individual who has elected hospice care receives general inpatient care in an inpatient facility for pain control or acute or chronic symptom management which cannot be managed in other settings. Inpatient Respite Care: When an individual who has elected hospice care receives care in an approved facility on a short-term basis to provide respite for a caregiver or absent caregiver. Memories - program provides specialized services for patients with Alzheimer?s or end stage dementia related diseases. Community education - certified education specialists offer a variety of educational programs to the community.
